A rain track is a few seconds of audio repeating for eight hours. Once you have heard the seam, you cannot unhear it — and at 3am you have nothing to do but listen for it.

Really. There are no audio files in the app to loop. Noise is generated sample by sample, and events like raindrops, thunder and crackles are scheduled at random times as they occur. Leave the same mix running for eight hours and no two minutes are identical.
Yes. It declares itself an audio app to iOS and runs a playback service on Android, so it plays all night with the screen off and appears on your lock screen.
No, and not as a policy choice — the app makes no network requests, so there is no channel through which anything could leave your phone. Put it in airplane mode and it behaves identically. Levels and saved mixes stay on the device.
